Overview

Primary Accountabilities:

The position is responsible for the day-to-day coordination of program activities and supervision of staff for the Parents as Teachers (PAT) program within the Family Services Department.  This position ensures that programs are implemented and operate in accordance with federal, state, and local standards, policies, and guidelines. This position is a working supervisor position with 50% of the time dedicated to providing direct patient services, home visitation services, and coordination of outreach activities such as health education, community participation, and other program activities, and 50% of the time dedicated to supervisory, administrative, and program management responsibilities.

Responsibilities

Description of Primary Responsibilities:

  1. Management of daily work activities and supervision of program staff.
  2. Responsible for managing schedules caseload assignment by grant source and assigning responsibilities and duties to employees in the department to provide services in an efficient and effective manner.
  3. Ensures employees receive instruction/training needed to perform work duties competently.
  4. Monitors’ performance as appropriate, provides mentoring/guidance to assist in employee development.
  5. Ensures that employees are aware of and adhere to all company and program policies and procedures.
  6. Conducts performance appraisals, completes performance appraisal documentation and meets with employees to provide feedback.
  7. Completes observations of home visits and program activities in accordance with PAT program requirements and guidelines.
  8. Responsible for successful implementation of programs.
  9. Implements program or clinic activities in accordance with applicable program guidelines/expectations to ensure program objectives are met and are compliant.
  10. Reviews work for thoroughness and accuracy and provide specific instruction to employees on improvement or corrective actions.
  11. Investigates and resolves complaints or refers unusual problems to the Director of Programs
  12. Collects data and provides regular updates and reports on program outcomes.
  13. Provides direct services and coordination of outreach activities.
  14. Provides direct services as needed for families participating in programs.
  15. Maintains records that document interventions provided in accordance with program requirements and guidelines.
  16. Collaborate with other clinic departments as well as community partners to assure that the needs of patients are being met.
  17. Ability to demonstrate a working knowledge of all Family Services programs and clinic services in the community and provide job coverage for other departmental staff when needed.
  18. Travels to support program activities and other work-related activities, i.e. required program meetings, home visits and/or trainings.

  4. Must always uphold confidentiality regarding CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.

Qualifications

Professional and Technical Knowledge:

  1. Possesses a basic level of written and verbal communication skills and mathematical knowledge typically acquired through completion of a bachelor’s degree and relevant work experience in management and supervision.
  2. Knowledge of early childhood development and program implementation and operations.
  3. Experience working with children and their families, as well as providing home visitation, family support services, and community outreach activities.
  4. Knowledge of Parents as Teachers and other evidence-based family support models preferred.

 

Licenses and Certification:

  1. Possesses a valid driver’s license.

 

Technical Skills:

  1. Ability to prepare basic correspondence and simple reports in Microsoft Word.
  2. Ability to use Microsoft Excel to create tables and simple displays of information.
  3. Ability to create basic presentations in Microsoft PowerPoint.
  4. Ability to perform basic e-mail functions using Outlook.
  5. Ability to enter and manage data into a centralized electronic health record and applicable program data system(s).

 

Communication Skills:

  1. Job duties require the employee to effectively communicate their opinions and extrapolations of information they collect and synthesize/analyze.
  2. Employees are required to negotiate resolutions to complex situations which may be inherently adversarial.
  3. Duties require the compilation and analysis of information prepared in effective written form, including correspondence, reports, articles, or other documentation.
  4. Duties involve convincing others to take actions or to behave in specific way desired by the employee.
  5. This position requires bilingual fluency, including reading, writing, and speaking in Spanish and English.

Blood/Fluid Exposure Risk: According to this position this is a Category III

 

  1. Category I: Tasks routinely involve a potential for mucous membrane or skin contact exposure to blood, fluids or tissue. Use of personal protective equipment (PPE), when appropriate, is required.
  2. Category II: Usual Tasks do not involve exposure to blood, body fluid, or tissues but may require performing Unplanned Category I tasks.
  3. Category III: Tasks involve no greater exposure to blood, body fluids or tissues than would be encountered by a visit. Category I tasks are not a condition of employment.

 

Physical Demands & Vision Requirements:

  1. Ability to work indoors and outdoors in various weather conditions.
  2. Ability to kneel, stand, walk, sit, use hands, fingers, and talk frequently.
  3. Ability to lift up-to 25 pounds.
  4. Close vision (clear vision at 20 inches or less).
  5. Distance vision (clear vision at 20 feet or more).
  6. Ability to adjust focus (ability to adjust the eye to bring an object into sharp focus).

 

Travel:

  1. Frequent travel to various clinic sites and other program functions is required.
  2. Frequent travel to other clinics, home visits, and off-site program functions is required.
  3. Possess a valid driver’s license and meet company insurance standards to drive company vehicles.
