Primary Accountabilities:
The position is responsible for the day-to-day coordination of program activities and supervision of staff for Family Services Programs such as Early Intervention Services and Outreach Programs. This position ensures that programs are implemented and operate in accordance with federal, state, and local standards, policies, and guidelines. This position is a working supervisor position with 50% of the time dedicated to providing direct patient services, home visitation services, and coordination of outreach activities such as health education, community participation, and other program activities, and 50% of the time dedicated to supervisory, administrative, and program management responsibilities.
Description of Primary Responsibilities:
1. Management of daily work activities and supervision of program staff.
2. Responsible for successful implementation of programs.
3. Provides direct services and coordination of outreach activities.
4. Other duties as assigned.
5. Devotes the time needed during the work week to ensure positive patient outcomes and day-to-day objectives are met.
6. Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior.
7. Must always uphold confidentiality regarding CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.
