Overview

Primary Accountability

 

The Population Health Coordinator is responsible to assist in designing and implementing sustainable State and Federal quality programs to improve the overall quality of care for our patients. The PH Coordinator will coordinate patient care activities for those in need of enhanced services: Chronic Care Management, appointment scheduling, health education and referral to services in order to close care gaps that are present. The PH Coordinator will ensure patients receive high quality healthcare and support services through use of care protocols, care plans, self-management goals and referrals to appropriate services.

 

 

Population Health Coordinator: $22.55p/h -$27.06p/h DOE with ability to go higher for highly experienced candidates.

Responsibilities

Description of Primary Responsibilities

 

  1. Designing processes and tools to reach out to patient populations overdue for preventative health services.
  2. Create and manage portfolios of patient registries and specific populations.
  3. Work with patients both in person and over the phone on improving their overall wellness and schedule necessary appointments.
  4. Gathers and organizes data to identify unmet needs, engage patients in care, assist in development of interventions, and enhance ongoing communication between patients and their care team.
  5. Assess patient needs, document and communicate to care teams for follow-up.
  6. Communicates population health information and data to health care teams and supports practice staff in developing creative processes to proactively manage target populations.
  7. Manages insurance assigned rosters, identifies patient outreach opportunities, and gets patients assigned/reassigned as needed.
  8. Utilizes EH and other data systems to retrieve and/or input patient demographics and health data.
  9. Composes and publishes clear and accurate reports pertaining to patient populations, as needed or directed.
  10. Demonstrates the ability to determine and track compliance with clinical quality measures.
  11. Collaborating and meeting with clinical care teams and working to create and implement plans of care for patients.  
  12. Communicating to patients regularly in regards to their necessary appointments, completing necessary documentation, as well as providing follow-up as necessary.
  13. Ensure timely documentation within the electronic health record and assist with data collection for specific clinical quality measures.
  14. Other Duties will be assigned upon hire.

Qualifications

Professional and Technical Knowledge:

  1. High School Diploma or equivalent required. Degree in Healthcare Administration, Public Health, Social Work, Business Administration, or related field highly preferred. Minimum 2 years of experience in customer service, preferably in healthcare setting.
  2. Knowledge of patient care process and clinical procedures and medical terminology.
  3. Working knowledge of quality improvement processes and optimal workflows.
  4. Registered Medical Assistant License or higher in good standing in the State of Washington or eligibility to be licensed as Medical Assistant Registered upon hire.

Technical Skills:

  1. Ability to effectively utilize and navigate Electronic Medical Record system.
  2. Ability to process prescriptions using the pharmacy management system.
  3. Ability to perform necessary task on the automated dispensing machine.
  4. Ability to process CII orders using the DEA Controlled Substance Ordering System.
  5. Ability to navigate wholesaler website and order medications when necessary.
  6. Ability to prepare basic correspondence and simple reports in Microsoft Word.
  7. Ability to use Microsoft Excel to create tables and simple displays of information.
  8. Ability to create basic presentations in Microsoft PowerPoint.
  9. Ability to perform basic e-mail functions using Outlook.

 

Communication Skills:

  1. Job duties require the effective communication of information during informal and formal verbal presentations.
  2. Job duties require the effective communication of information in written (including electronic) correspondence.
  3. Communicates in a clear and effective manner.
